What the Program Offers

The GPD Case Management Program is designed to help veterans maintain housing and continue building independence after moving into a permanent home. Support is provided in the community and is based on each veteran’s current needs, goals, and available resources.

The GPD Case Manager may help with practical areas that can affect long-term housing stability, including communication with landlords, budgeting, independent living skills, and connecting to additional services when needed.

Support That Meets Veterans Where They Are

Veterans who recently moved into permanent housing may still need guidance as they adjust to living independently. This program provides case management outside a residential setting, allowing support to take place where veterans live and manage daily responsibilities.

The goal is to help each veteran strengthen the skills and support systems needed to remain housed.

Tenant and Landlord Relations

Support with communication, housing expectations, and resolving basic concerns that may come up after moving into permanent housing.

Independent Living Skills

Guidance with everyday responsibilities can help veterans feel more confident and prepared in their housing.

Budgeting & Financial Support

Help with basic budgeting habits, planning for monthly expenses, and managing financial responsibilities related to housing. Veterans also receive financial support for housing needs after they have been in the program for 30 days.

Community-Based Case Management

Individualized support is provided outside a residential program, tailored to the veteran’s current situation and needs.

This program may be a good fit for veterans who have recently transitioned into permanent housing but do not have ongoing case management through another provider.

It is intended for veterans who may benefit from continued support as they work toward housing stability and greater independence.

GPD Community Case Management FAQ

GPD Case Management is a community-based program that provides individualized support for veterans who have recently moved into permanent housing.

It is for veterans who have recently transitioned into permanent housing and are not currently receiving case management through another provider, such as HUD-VASH or SSVF.

Support may include help with tenant and landlord relations, independent living skills, budgeting, and other needs related to maintaining housing stability.

The GPD Case Manager is community-based, so support is provided outside of a residential setting.
